[Solved] Conditional Formatting Regularly gets garbled

I have 2 rows at the top, and then a table underneath. I say that because it keeps me from adding conditional formatting all the way to the top in some columns.
For each column, I have 2 types of formatting. One includes a TOP and a BOTTOM and the other has 3 types of formulas =B4>B3, =B4<B3, and =B4=B3. 

I arrange them by columns, B-F. For column B, I start with the TOP, then the BOTTOM, and then, the 3 formulas for that column. In Column B, one of the top 2 rows has a number in it, so, the "Applies To" is =$B$4:$B$274. I have 274 rows in the table. I've tried using the max row, 1 more than the max ro (275) and even tried 1000 or more. Nothing helps.

It seems to work for a while and then, wham, everything in the lists seems to be garbled and I have to duplicate those that work, adding it to a garbled column, up and down....

I've fought with this for months -- What can possibly cause this?

Could you please upload your file, demonstrating the issue? Remove any confidential information but leave the structure of the workbook/sheets untouched.

I didn't know whether or not uploading an xlsx file would work here, so I included a Zipped version too. Also - I am using Office 365 if that makes any difference to you.
BTW - everything in this version was just fixed before I posted this, and is working as it is.

One more thing -- it gets garbled after adding multiple lines to the table. I don't have an exact scenario (like how many line additions), though

I believe you only need five rules for the entire table. No need for a set of rules for every column. The CF rules in the attached file seem to work OK. Also

when rows get added or deleted.
