How do you combine several IF functions together when there are 2 answers ???

In the attachment I have a formula that combines IF with AND

My problem is this -
I have several conditions - the problem is that each condition individually has both an answer - Value if true and an answer to Value if false
So I can not insert another IF within the Value if false argument

The different conditions are written in the Reference Table - each row is a separate condition

I set it up with OR at the beginning of the formula and failed
How to do it ??

I tried to find out 'The different conditions are written in the Reference Table - each row is a separate condition'. I must admit that I didn't understand what Excel was supposed to do. Could you be more explicit? Thank you,
