I can only assume that there are duplicate data in your file, but this is just an assumption.
You can always use the Remove duplicates after the merge.

Hello Anders, You guessed correctly. Since I use the Left outer which has a concept of Many to One or One to One relationship, yesterday after posting this query I analyzed my files with data having multiple entries and filtered them. Then tried to identify the common thing and all belonged to a common file which I used for meging. On reviewing the file I identified duplicate entires for those listed accounts and after removing the duplicate entries the issue got resolved.
