The attached file has two tables. The first is how the data will be received. The second is how it will need to be formatted.

Please have a look at the attached. It's your file with a simple PowerQuery (PQ) solution in it.
Note that I formatted the first table as a structured Excel table. These don't allow duplicate column names so the fourth column has a "2" added to it. That's fixed in a later step, though. Are you familiar with PQ?

I have not used Power Query. Will one of Mynda's videos cover enough to do what I need?
