Hi,
Please see the attached file which contains INDEX & MATCH formulas that lookup the various criteria individually and then find the maximum cost, which I presume is how it works. You could consolidate these formulas into one cell, but I've left them separate so you can follow the logic.
You'll need to factor in the order handling fees and cost for every lb over 3lbs, but that should be fairly straight forward.
